5mm LEDs are diodes contained within a 5mm lens with two thin metal legs on the bottom. They are used in applications that require a low amount of light. 5mm LEDs also have much lower drive currents, with a maximum of around 30mA, whereas Surface Mount LEDs require at least 350mA.

Surface Mount LEDs are diodes that can be placed on a substrate (circuit board) and are protected by a silicon dome.

Electronic polarity indicates whether or not a circuit is symmetric. Because LEDs are diodes, current can only flow in one direction.
